First Holy Communion Registration

This programme is for parishioners of Our Lady of Grace and St Edward’s, Chiswick, and St Dunstan’s, Gunnersbury (including those who live outside of the parish but regularly worship here) and St Joseph’s, Grove Park.

If your child is in year 3 or above and/or born before 31st August 2012 and you would like your child to receive the Sacrament of Holy Communion, and you can commit to all the sessions highlighted in the 2019-20 Programme, please complete the below Registration Form.

The closing date for registration for the 2019-20 programme is

8th September 2019

We ask you for a £25 offering towards to costs of the programme, including course materials.

If this is a hardship for you, then a lesser amount would be acceptable.

Please submit the original baptismal certificate to the Parish Office before 8th September. All original certificates will be returned to you (photocopy/fax/email/pdf not acceptable). Posting is not recommended.

Corpus Christi Procession
To celebrate Holy Communion all the children who make their First Holy Communion are invited to wear their First Holy Communion outfits to lead the Corpus Christi procession which will take place immediately following the 12.15pm Mass on Sunday 14th June 2020.
Your Commitment and Support of the Program *
I have read the course content and I agree to take a full part in the preparation of my child for Reconciliation and First Holy Communion. I will ensure that my child attends all sessions with the catechists, attends Sunday Mass, completes the homework from the course books and learns the prayers set in the Welcome Pack.
Protecting your privacy:

Next steps:
Once you have completed this form, please collect a First Holy Communion Welcome Pack from the Parish Office, where you can also submit the £25 course offering, a copy of your child's original Certificate of Baptism, and an optional passport size photo of your child (which will be displayed in church around the time of the First Holy Communion Masses, on a 'Holy Communion Prayer Vine'. The children will NOT be named on this display) to the Parish Office by Sunday 8th September 2019 marked for the attention of ‘First Holy Communion’. If you do not wish to have your child's photo displayed then please do not supply a photo. Supplying a photo consents to it being displayed in church as part of the display.
