If you would like your child to do his/her First Holy Communion and First Reconciliation (Confession), please complete the Registration Form provided below

REQUIREMENTS TO ENROL YOUR CHILD:

The Child must live within the parish boundaries of

Our Lady of Grace and St Edward’s, Chiswick,
St Dunstan’s, Gunnersbury
St Joseph’s, Grove Park
and/or have these churches as his/her common place of worship.
2. Your child is in year 3 or above and/or born before 31st August 2018
3. You can commit to all the sessions highlighted in the 2025-26 Programme

The closing date for registration for the 2025-26 programme is:

Sunday 5th October 2025

We ask you for a £30 offering towards to costs of the programme, including course materials.

If this is a hardship for you, then a lesser amount would be acceptable.

Please submit the original baptismal certificate to the Parish Office by the deadline in the Welcome Pack. All original certificates will be returned to you. Photocopy/fax/email/pdf are not acceptable without an acceptable reason. Please be note that without proof of your child's baptism by Church Law we CANNOT administer the sacrmaents of Reconciliation or Holy Communion.

NEXT STEPS

Once you have completed this form, please collect a First Holy Communion Welcome Pack from the Parish Office. At the same time you can also submit the £30 course offering and a copy of your child's original Certificate of Baptism.
